Blood Compliance Report 2018
The blood compliance report and declaration forms for Hospital Blood Banks and Blood Facilities (01st April 2017 to 31st March 2018) are now available on the MHRA website using the links below and must be completed and submitted to bcr@mhra.gov.uk by 30th April 2018.

Please note that late submissions increase the risk score applied to the site.
For hospital blood banks, it is a requirement to complete the compliance report and the declaration form. A change in approach to blood compliance report assessment was introduced during 2015 which saw a removal of the majority of questions requiring free-text responses. You will note that some sections are not available or some question numbers do not appear to be in order in some sections. Please ensure all the available questions on the BCR are answered and submit your completed BCR as outlined in the guidance notes.
For blood facilities, the blood facility declaration form must be completed and sent to the MHRA.

Facilities Declaration Page
Please can someone clarify the following, just a little confused as I am unsure this was completed correctly previously:
If a HBB supplies blood to another hospital – deemed a facility only because they manage their equipment , and there is a technical agreement in place.
Does the facilities declaration page need to be signed by the following:
1. Staff at the facility completing facility the details
2) Person overall responsible for the facility
3) The CEO of the hospital Blood bank that supplies the facility (deemed the Third party service provider)?
