Would be good to discuss why some sites, particularly those with full electronic vein-to-vein systems that have been in place for many years, are still finding it difficult to achieve 100% (or even close to this figure ) traceability every month, considering these systems cost upwards of £500K +?

What are the main obstacles? This information would be really helpful for sites planning implementation.
